چکیده

A78-year-old woman presented with a 2-month history of worsening dyspnea, orthopnea, and cough with hemoptysis. Her past medical history was significant for hypertension and severe mitral regurgitation for which she had undergone mitral valve repair 3 years previously. In addition, she had recurrent atrial tachycardia requiring 2 radiofrequency ablations, which included ablation of several left-sided pathways. The last ablation was performed 3 months before her presentation. A transesophageal echocardiogram was performed and showed turbulent Doppler flow and increased systolic and diastolic velocities in the right lower (Figure 1) and left upper pulmonary veins. A contrast-enhanced, retrospectively gated helical CT scan showed 60% to 70% stenoses of the right lower (Figure 2, left panel) and left upper pulmonary veins. A ventilation-perfusion scan revealed a large perfusion defect in the right lower lobe. The patient subsequently underwent a successful balloon dilation of the right lower and left upper pulmonary veins (Figure 3). Pulmonary artery pressures before dilation were 85/30 mm Hg, and dropped to 55/ 20 mm Hg immediately after dilation. A transesophageal echocardiogram performed after the procedure showed normal flows in the previously stenotic pulmonary veins. A CT scan completed several days after the procedure showed a reduction in the degree of stenosis in both right lower (Figure 2, right panel) and left upper pulmonary veins. The patient’s acute symptoms were markedly improved, although her condition subsequently deteriorated secondary to multiple other medical problems.

Pulmonary Vein Stenosis Mimicking Nonspecific Interstitial Pneumonia

Pulmonary vein stenosis (PVS) is a known complication after catheter ablation of arrhythmias. Surprisingly, little information is available on its manifestations in the lung. We describe the case of a 39-year-old woman who presented from an outside hospital with worsening shortness of breath after catheter ablation of pulmonary veins for atrial fibrillation. After an initial diagnosis of pneumo...

Incidence of pulmonary vein stenosis 2 years after radiofrequency catheter ablation of refractory atrial fibrillation.

AIMS Pulmonary vein ablation offers the potential to cure patients with atrial fibrillation. In this study, we investigated the incidence of pulmonary vein stenosis after radiofrequency catheter ablation of refractory atrial fibrillation by systematic long-term follow-up. METHODS AND RESULTS Forty-seven patients with refractory and highly symptomatic atrial fibrillation underwent radiofrequen...

Catheter cryoablation of cardiac arrhythmias.

PURPOSE OF REVIEW The purpose of the review is to provide an update on the safety and efficacy of catheter cryoablation. RECENT FINDINGS Catheter cryoablation is a safe and clinically effective method for ablation of atrioventricular nodal reentrant supraventricular tachycardia. Although the acute procedural success rate of catheter cryoablation for this arrhythmia may be slightly lower than ...

Pulmonary vein stenosis after second-generation cryoballoon ablation for atrial fibrillation

Pulmonary vein isolation (PVI) is the cornerstone of catheter ablation for atrial fibrillation (AF), especially in paroxysmal 1 AF patients. In addition to a radiofrequency ablation catheter, a cryoballoon has been proven as an effective technology in PVI for AF. Recently several randomized trials have shown that cryoballoon ablation was noninferior to radiofrequency ablation with respect to ef...

Massive hemoptysis due to pulmonary vein stenosis following catheter ablation for atrial fibrillation.

Pulmonary vein stenosis, which is one of the rare complications of radiofrequency catheter ablation for atrial fibrillation, has various symptoms. Here, we report a rare case of massive hemoptysis due to pulmonary vein stenosis following radiofrequency catheter ablation, which was successfully managed with pneumonectomy.

CARDIOVASCULAR MEDICINE Pulmonary vein ablation for idiopathic atrial fibrillation: six month outcome of first procedure in 100 consecutive patients

Objectives: To report six month outcome in patients undergoing their first pulmonary vein ablation procedure for idiopathic atrial fibrillation (AF) at a ‘‘non-pioneering’’ hospital. Design: Prospective observational study. Setting: Specialist electrophysiology unit at a university hospital. Patients: The first 100 consecutive patients undergoing their first pulmonary vein catheter ablation pro...
